Queens Center for Rehabilitation & Nursing is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. This is an excellent opportunity for a caring professional who is passionate about supporting residents and their families while helping them navigate their healthcare journey in a skilled nursing and rehabilitation setting.

As a Social Worker, you will play a vital role in promoting residents' emotional well-being, coordinating services, and ensuring a positive experience throughout their stay.

Responsibilities

• Complete psychosocial assessments for new admissions and ongoing resident evaluations.

• Develop and implement individualized care plans in collaboration with the interdisciplinary team.

• Provide emotional support, counseling, and crisis intervention for residents and their families.

• Coordinate discharge planning to ensure safe and successful transitions.

• Assist residents and families with community resources, financial assistance, and advance care planning.

• Advocate for resident rights and promote dignity, independence, and quality of life.

• Participate in interdisciplinary care plan meetings and family conferences.

• Maintain accurate documentation in accordance with CMS, state, and facility requirements.

• Collaborate with nursing, rehabilitation, admissions, and medical staff to support resident-centered care.

• Assist with resident adjustment, conflict resolution, and grievance management.

• Support regulatory compliance and survey readiness.

Qualifications

• Bachelor's degree in Social Work or a related Human Services field required.

• Social Work experience in a Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, or healthcare setting preferred.

• Knowledge of discharge planning, resident rights, and CMS regulations preferred.

• Excellent communication, interpersonal, and organizational skills.

• Ability to work collaboratively within a multidisciplinary team.
