Details:
Job Duration: 13 weeks

Shift: Day 5x8-Hour (08:00 - 16:30)

Patient Age Groups

Adults, Geriatrics

Daily Responsibilities:

Develop and implement care plans for patients
Provide skilled nursing care and interventions
Administer IV therapy and medications
Assess patient needs and vital signs
Collaborate with healthcare team to ensure quality patient care

Required Skills:

Inpatient Acute Case Management (Case Manager Hospital experience)
MUST HAVE DISCHARGE PLANNING EXPERIENCE
BLS, CA RN License
