LPN Case Management Support

divh2Licensed Practical Nurse (LPN)/h2pWe are seeking a Licensed Practical Nurse (LPN) to join our team and work closely with our case management staff to support coordinated, patient-centered care./ppKey Responsibilities:/pulliAssist with patient assessments and monitoring/liliCollaborate with case managers and care team members/liliPerform medication set-ups (med-set ups) and administer medications as ordered/liliDocument patient status and report changes/liliEducate patients and caregivers on care plans and medications/li/ulpQualifications:/pulliActive LPN license in good standing with the Indiana State Board of Nursing/liliCompliance with Indiana licensing requirements (IPLA), including renewal and scope of practice/liliStrong communication, organization, and teamwork skills/liliExperience in home health or case management preferred/li/ulpJoin our team to make a meaningful impact through compassionate, coordinated care.
