CVS Health is seeking an A1A Case Manager RN to coordinate holistic care for members across acute, post-acute, and community settings. In this role, you will perform telephonic and/or virtual assessments, develop individualized care plans, and collaborate with providers, payors, and caregivers to optimize outcomes and reduce readmissions. You will educate members on conditions and medications, close care gaps, and document in clinical systems using evidence-based guidelines. You’ll work within a supportive, inclusive culture that values integrity, continuous learning, and opportunities for career growth nationwide.
