Hospice RN Case Manager: Compassionate Care Leader

Mount Carmel Home Care & Hospice in Columbus, OH is seeking a compassionate Hospice RN Case Manager to join our interdisciplinary team. You will coordinate care for hospice patients and families, develop individualized care plans, and manage symptoms with evidence-based hospice practices. You will supervise aides and LPNs, ensure regulatory compliance, and provide patient-centered education and emotional support throughout the hospice journey. #J-18808-Ljbffr
