Hospice Registered Nurse Case Manager

Transitions Hospice

Transitions Hospice - Ohio is seeking a dedicated RN Case Manager to plan, organize, and direct hospice care in patients' homes within Franklin County and surrounding areas. You will collaborate with an interdisciplinary team to tailor services to patient and family needs. The role requires an RN license in Ohio, nursing degree, and hospice or home health experience is preferred.
