Job Description
Case Management - Medical Social Work

Schedule

Weekend requirement: Every other weekend. Holiday coverage is required; must commit to working 3 of the 5 core holidays. Orientation lasts approximately 40 hours.

Assignment Details

Caseload of up to 25 patients. Must be available for a travel assignment; prior travel experience is necessary.

Requirements

• 2 years of acute hospital case management or medical social work experience.
• Valid state license required; compact license must be in hand at the time of submission.
• Experience with discharge planning, bio-psychosocial assessments, crisis intervention, and care coordination.
• Familiarity with documentation requirements in a computerized system.
• Experience with EMR systems, specifically Epic, is required.

Responsibilities

• Coordinate care and facilitate discharge planning.
• Conduct bio-psychosocial assessments and participate in treatment planning.
• Collaborate with multidisciplinary teams including PT, RT, pharmacy, and social services.
• Educate patients and caregivers on treatment plans and available resources.
• Handle referrals for home care, rehabilitation, and long-term care services.
• Participate in family meetings and crisis interventions as necessary.

Compliance / Notes

A maximum of 7 days of requested time off is allowed; must be submitted up front during submission. Providers may be expected to float to nearby facilities.
