Clinical Direct Care Counselor – Brooklyn & Staten Island Full Time Regular, Brooklyn, NY, US Requisition ID: 1274 Salary Range: $18.20 to $18.20 per hour Locations: Brooklyn & Staten Island. Responsibilities Assist the facility nurse and psychologist with providing medical services to individuals. Coordinate medical appointments (audiology, dentistry, neurology, ophthalmology, physical examinations, laboratory work, psychiatry, physical therapy, occupational therapy, and other services as needed). Schedule and attend clinic appointments; communicate results and any medication changes to the nurse, psychologist, and/or residence manager; post follow‑up appointments on the clinic calendar. Serve as the liaison between the nurse, psychologist, residence manager, and assistant manager, ensuring all parties have timely information. Advocate for the best possible care for individuals at clinics, hospitals, and doctor’s offices. Prepare clinic consults and required paperwork in the absence of the nurse. Maintain sufficient medication supply and equipment when the nurse is absent. Use Precision Care software to maintain and track individuals’ medical information. Communicate with day programs regarding medical appointments and emergency visits. Inform management when additional supports are needed for medical appointments. Maintain bowel movement charts, menstrual cycle sheets, and other medical monitoring systems under the direction of the nurse. Maintain and write monthly MAR sheets under the direction of the nurse. Perform any additional tasks as designated by the residence manager, nurse, or psychologist. Lift and transfer up to 25 pounds; operate Hoyer lifts and use wheelchairs or walkers as required. Qualifications Valid driver’s license. High school diploma or GED. Proficient reading, writing, and effective communication skills. Knowledge of basic medical terminology. Good organizational and time‑management skills. Basic computer skills. Valid AMAP certification. Required Training & Certifications Successfully complete First Aid, CPR, and AED training. Complete AMAP training and certification within 60 days. Complete SCIP training within 30 days. Attend in‑house shadowing and orientation. Adhere to COVID‑19 protocols: PPE, daily screenings, hand hygiene, surface disinfection, and social distancing. Physical Demands Ability to lift and transfer up to 25 pounds.
