As a Nurse Practitioner supporting Home Health, Hospice, and Palliative Care , you will:

• Conduct patient visits across all three service lines
• Collaborate and participate in weekly interdisciplinary group (IDG) meetings
• Provide clinical guidance and continuity of care
• Play a key role in shaping patient-centered outcomes

Schedule Highlights

• Flexible, mission-driven work environment
• 4 weeks per year: Monday-Monday coverage rotation

Requirements:

• Graduate of an approved Nurse Practitioner curriculum
• Active APRN or NP license required
• Minimum of two years clinical nursing experience
• Valid driver's license
