As a Licensed Mental Health Counselor (LMHC), you will be responsible for providing high-quality behavioral health services to residents of long-term care and rehabilitation centers. Our clinicians are dedicated to improving the quality of life for our older adult patients, and we are seeking a like-minded professional to join our team. The ideal candidate will have a strong foundation in counseling theories and techniques, with experience working with older adults or individuals with mental health conditions.
Responsibilities:
- Provide individual and group counseling sessions to residents of long-term care and rehabilitation centers
- Develop and implement personalized treatment plans to address patient-specific needs and goals
- Collaborate with interdisciplinary teams to ensure comprehensive care and support
- Conduct assessments and evaluations to identify mental health needs and develop treatment plans
- Maintain accurate and thorough records of patient interactions and progress
