Registered Nurse - Long Term Care

Job Summary Responsible for providing skilled nursing care to residents according to established care plan and in compliance with federal and state regulations. Provides guidance and direction to ancillary staff. Responsibilities Provide quality care to acute, subacute, and chronically ill residents; administer medications and treatments according to standards; maintain competency in required nursing skills. Provide direction to CNAs and monitor workflow to ensure coordination of care; use nursing process for assessment, diagnosis, planning, intervention, and evaluation; report significant changes to physician as indicated. Maintain accurate inventory of medications at shift start and end; ensure accurate distribution according to the 5 rights; monitor side effects; ensure medication provision consistent with manufacturer's guidelines. Give guidance to auxiliary staff; participate in report meetings; communicate with residents, guardians, and family about condition and needs; provide teaching/training to residents and families as needed. Ensure preventative measures: specialty mattresses, foam boots, weekly skin checks, turning/repositioning schedules, toileting schedules, cushions for all wheelchairs; follow up on reports of rashes, open areas, edema, blisters, skin tears, or bleeding. Respond promptly to alarms; ensure restraint alternatives activated: personal alarms, cushion alarms, wheelchair belts, tumble mats, alarm mats, bolstered beds, hip protectors. Provide and ensure quality end‑of‑life care; use comfort basket; meet resident and family needs; ensure hygiene, comfort, spiritual needs; treat pain/restlessness; offer family courtesy meal trays and amenities; demonstrate compliance with post‑mortem care; participate in departure ceremony. Work in a constant state of alertness and safe manner. May perform point‑of‑care testing according to policies and procedures. Perform other duties as assigned. Education Graduate of accredited school of nursing or education equivalency for licensing. Experience No experience required. Physical Requirements Constant use of speech to share information through oral communication. Constant standing and walking. Frequent lifting/carrying and pushing/pulling objects weighing 0‑25 lbs. Frequent sitting, reaching and keyboard use/data entry. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. Frequent use of smell to detect/recognize odors. Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc. Occasional lifting/carrying and pushing/pulling objects weighing 25‑50 lbs. Occasional lifting/moving of patients. Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements. Occasional driving. Rare crawling and running. Licensing Requirements Illinois: Basic Life Support HealthCare Provider (BLS HCP) – American Heart Association (AHA) AND Registered Professional Nurse (RN) – Illinois Department of Financial and Professional Regulation (IDFPR).
