SOCIAL WORKER (MA/LMSW) - Fixed-term

Fixed Term - Social Worker (LCSW-A) - Behavioral Health – Glendale, CA Horizon Health Corporation is seeking a Fixed Term Social Worker (LCSW-A) in Glendale, CA . The Social Worker I is responsible for providing high quality patient care, displaying good skills in assessment, guidance and discharge planning. The Social Worker I assists patients and their families in understanding and adjusting to diagnoses and potential treatments. The Social Worker I can also help identify resources during treatment and after discharge. The Social Worker I effectively participates as a member of the multidisciplinary team and the unit community relations team. The Social Worker I may be under the direct clinical supervision of a Social Worker II. Responsibilities At all times, conducts oneself ethically in accordance with all relevant standards and regulations Maintains Knowledge of and demonstrates respect for patient rights Works sufficient hours to ensure effective performance. Coordinates work hours with supervisor Implements a treatment plan of care for patients based on a thorough assessment of the patient’s symptom presentation, professional/referral input and all other available data Appropriately participates in referral development processes and activities as directed/requested Actively involved in assessing patient progress and communicating results and trends to supervisor and treatment team As directed, conducts individual psychotherapy sessions in accordance with state/regulatory standards, displays sufficient theoretical knowledge of psychopathology and effective treatment techniques (under the guidance of a LCSW) As directed, conducts family therapy sessions in accordance with state/regulatory standards, displaying sufficient theoretical knowledge of family therapy principles/psychopathology and effective treatment techniques As directed, conducts group therapy sessions, displaying good theoretical knowledge of group therapy principles/psychopathology and effective group therapy techniques. As directed, conducts activity therapy sessions, displaying good theoretical knowledge of activity therapy principles/psychopathology and effective group therapy techniques. As directed, conducts educational group sessions, displaying good theoretical knowledge of subject matter and effective communication skills in presenting material Provides group and individual therapies on weekends as directed by supervisor Conducts therapies in accordance with patient diagnosis and needs Provides ongoing education to patients and families as needed. Provides appropriate resources and support for patients before, during, and after treatment. Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician. Displays thorough knowledge of referral/placement process and available community resources and makes effective contact with disposition sources Completes progress notes and assessments according to program procedures Participates as an active member of the interdisciplinary and treatment teams Submits timely and accurate CQI+ outcome data on a weekly basis Actively involved in measuring patient performance; communicates results and trends to supervisor Provides feedback to supervisor on an ongoing basis in regard to concerns, improvements, changes, etc. Attends Unit and Hospital meetings as directed Identifies areas needing improvement and utilizes the facility performance improvement process Actively participates in department processes as requested Ensures facility is safe and reports deficiencies to the Program Director Ensures confidentiality of all information encountered Maintains professional interactions with peers, senior management, and staff through effective written and verbal communications Assists with ambulating and lifting patients as needed Completes and maintains all required state/regulatory, hospital and company competencies and training expectations Participates as an active member of the Referral Development Team If applicable, serves as the Coordinator of the IOP/OP Service line If applicable, assigned to the Inpatient Program(s) If applicable, assigned to the IOP Program(s) If applicable, assigned to the PHP Program(s) If applicable, assigned to the OP Program(s) If applicable clinically supervised by an approved SW supervisor who is approved by Program Director or SW Director Other duties as assigned Benefit Highlights for full‑time positions Competitive Compensation Excellent Medical, Dental, Vision, and Prescription Drug Plan 401(K) with company match and discounted stock plan Long and Short‑term Disability Flexible Spending Accounts; Healthcare Savings Account Life Insurance Career development opportunities within the company Tuition Assistance Rewarding work environment – Enjoy going to work every day! For more information on this position, email Courtney Elbe, Horizon Health Corporation, Recruiter at View email address on click.appcast.io Qualifications Education, Certification/Licensure, and/or Experience A Bachelor’s Degree from an accredited college with a major in Social Work, Psychology, Sociology or similar field Must have a valid provisional state license in Social Work (LCSW‑A) as required by the state in which the Social Worker will be providing services; or as allowed by the governing state, the ability to work under the guidance of LCSW (Licensed Clinical Social Worker) or similar field. Preferred, minimum one year internship or experience in a similar position and/or industry Enhances professional growth and development through participation in educational programs, in‑service meetings and workshops. Keeps up to date on professional literature Maintain effectiveness and certification in required patient management protocols. Completes and maintains all required state/regulatory, Hospital and Horizon competencies and training/education expectations on a timely basis. Is knowledgeable of hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental and infection control standards Develops basic knowledge of Hospital and Horizon senior staff and organizational structure Knowledge, skills, and abilities required Proficient in software applications Effective presentation skills Ability to manage stress appropriately Ability to make decisions under pressure Ability to manager anger, fear, hostility and/or violence of others appropriately Maintains a positive attitude/public image while in the facility and in all work‑related activities outside the facility Language Skills Excellent written and verbal communication and presentation skills Ability to read and comprehend moderate to complex instructions and correspondence Ability to write detailed correspondence Ability to effectively present information in one‑on‑one and small group situations to patients and their families, Program leadership, employees, clients, providers, vendors, etc. Competencies Communicates effectively with other professional and support staff to achieve positive patient outcomes Communicates effectively with families, agencies and referral sources conveying a positive and professional image Ensures confidentiality of all information encountered Maintains professional interactions with peers, senior management, and staff through effective written and verbal communications Displays competent, effective interviewing and report‑writing skills Completes competent, clear progress notes and assessments according to program policies and procedures Work Environment This job operates in a professional office environment and/or clinical environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This position may have contact with patients who have moderate to severe psychiatric concerns and/or behavioral issues. There is the possibility this position could be exposed to violent behavior from patients and/or behavioral issues as part of their daily duties.
