Ready to join a supportive team that values your skills and experience?As an In-House Nurse Practitioner specializing in Infectious Disease, you will be the primary provider helping your facilities clinical expertise in their Infectious Disease programs by participating in appropriate resident's plan of care. You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.At Trio Health Partners, we focus on preventing critical health issues before they arise, delivering proactive care to residents in our Skilled Nursing and Long-Term Care facilities.Job Requirements:Active Illinois Nurse Practitioner License (unencumbered).Master's Degree in Nursing from an accredited program.Strong clinical background in Infectious disease.Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.Ability to engage and communicate effectively with elderly and chronically ill patients and their families.Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.Key Responsibilities: As the In-House Nurse Practitioner specializing in Infectious Disease, you will provide essential care and leadership within the facility, including:Performing comprehensive physical examinations and evaluating changes in patient conditionsPrescribing medications and treatments as neededDeveloping patient care policies and facilitating referrals to specialistsCoordinating advanced care planning and discharge visitsEnsuring timely communication and documentation of patient assessments, treatment plans, and follow-upsSupporting the physician-patient relationship and addressing gaps in careOur NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.Salary & Compensation:Annual Pay: $105,000 - $130,000 based on experienceMonthly Bonus Opportunities: Monthly performance-based bonusesBenefits:Health, Dental, and Vision Insurance401(k) plan22 days of Paid Time off annuallyCME Stipend and CME PTOMalpractice & Tail Insurance coverage providedApply today and make a difference in the lives of our patients and their families!
