Role and Responsibilities

1. Ensure quality and safe delivery of home care services.

Participates in the development and implementation of client plans of care per home care agency policy and procedure, as appropriate.

Participates in client case conferences according to home health care agency policy and procedure, if requested.

Information regarding client care plans is submitted to the Home Care SUPERVISOR promptly.

1. Implements current Certified Nurse Aide services.

-Service plans of care are regularly discussed with CAREvity’s supervisor.

-Client assignments and reports are received from CAREvity’s Supervisor.

Qualifications and Education Requirements.

-High school graduation or GED required.

-Certified Nurse Aide certification is required and obtained through successfully completing an approved program.

-Must have three years of full-time or part-time experience in personal care or an institutional setting, such as a hospital or nursing home, or must have three years of full-time or part-time experience within the last five years in direct client care in a home care agency setting.

-Evidence of sympathetic attitude toward care elderly.

-Demonstrated ability to read, write, and carry out directions.

-Evidence of maturity and ability to deal effectively with job demands.

-Good verbal and written communication skills are required.

-Attends (8) hours of personal care-oriented in-services per year.

-Participates in professional meetings when directed.

-A criminal history check should be conducted before being offered work with this agency.

-Ability to work closely supervised to ensure competence in providing client care.

-Must have your own car.

Physical Demands

-Must be able to read & write English.

-Must be physically able to lift, bend, and stand for long periods of time.

-Must be able to handle stressful situations due to the client’s medical conditions.

-Must adhere to assigned and agreed-upon work schedule.

-Must be able to drive up to 50 + miles per day.

-Must maintain a professional appearance and good personal hygiene.

-Must agree to a Competency Testing.

-Must have a five-year work history.

Certified Nurse Aides must pass a competency examination with at least an 80% or better. The content of the competency evaluation of the Agency will include and is not limited to: (if applicable)

-Must be able to work every other weekend; this is a requirement.

-Must possess the ability to operate a smartphone.
