Hospice Care Clinical Care ProviderAssumes the responsibility for identifying patient family needs and for providing clinical care in accordance with physicians orders and plan of care within the policies and procedure of Hospice CareEducationTechnical or Vocational school plus up to 2 years specialty trainingExperienceOne year of related experience. Requires a working knowledge of standard practices and procedures.Licenses & CertificationsDriver's License Georgia Licensed Practical Nurse or Multistate Nursing License Healthcare Provider (CPR) required within 30 days of hire. Vehicle InsuranceQualificationsA minimum of one (1) year experience as a LPN in an acute care setting preferred. BLS required within 30 days of hire. Dependable automobile Experience in Hospice preferred BLS required within 30 days of hire. Tanner Health System
