A rewarding opportunity is available for an experienced registered nurse specializing in home health care. This full-time, 13-week contract involves delivering high-quality nursing services directly to patients in their homes, using your clinical expertise and compassionate approach. You will play a vital role in patient care coordination and education while enjoying a supportive work environment.

Schedule & Setting:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based work with daily travel within your assigned territory (mileage reimbursement provided)
• Expected caseload: 30 points per week (typically 35 patient visits per day)

Key Qualifications:

• Active RN license in the state of practice and in good standing
• Minimum of 1 year of recent field-based home health nursing experience
• Working knowledge of OASIS assessments and Home Care Home Base (HCHB) documentation
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Perform comprehensive OASIS assessments to develop individualized care plans
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and management of chronic conditions
• Monitor patient progress, address changes in condition, and communicate promptly with healthcare teams
• Educate patients and families on diagnoses, treatment plans, medications, and preventive care
• Maintain thorough, timely documentation in Home Care Home Base (HCHB)
• Collaborate closely with therapists, social workers, and home health aides to ensure team-based, coordinated care
• Participate in case conferences, staff meetings, and ongoing training
• Uphold all regulatory and professional standards for home health nursing

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
