Concierge Home Care is seeking a motivated Home Health Account Executive to develop referral partnerships, increase patient admissions, and serve as the trusted resource for physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare providers throughout Bay and Gulf Counties.

What You'll Do

• Develop and maintain relationships with physicians, hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, case managers, and other referral sources.
• Increase home health admissions and achieve monthly sales goals.
• Execute strategic territory management plans.
• Educate referral partners on Concierge Home Care's services.
• Coordinate smooth patient transitions from hospital or facility to home.
• Serve as a trusted healthcare resource within the community.
• Collaborate closely with clinical leadership to ensure outstanding patient outcomes.
• Maintain CRM documentation and referral activity.
• Represent Concierge Home Care at networking events and community outreach opportunities.

Qualifications

Required

• Healthcare sales experience or strong clinical experience with existing healthcare relationships
• Excellent communication and relationship-building skills
• Self-motivated with the ability to work independently
• Valid driver's license
• Reliable transportation
• Auto insurance

Preferred

• Existing referral relationships within Bay or Gulf County
• Experience in:
