Foundation Medical Partners Home Care Affiliated with Southern New Hampshire Medical Center, Foundation Medical Partners is the second largest multi-specialty group in New Hampshire and serves the greater Nashua community. Our Home Care program is designed for patients who are unable to leave their home due to a medical condition. As a patient of Southern New Hampshire Health and Foundation Medical Partners, we are pleased to offer Home Care visitation services to support patients care while medically necessary. Our Nurse Practitioner is available to visit patients upon discharge from the hospital or on an ongoing basis. They will work collaboratively with primary care and/or specialists as well as patients care teams to provide care for specific needs. Our goal is to allow patients to remain in their homes but still receive the important health care needed! About the Job Responsible for greeting patients, answering all incoming telephone calls, providing general information, check-in and check-out for all patients, communicating with internal and external customers, scheduling appointments, entering and tracking referrals, filing, and other duties as assigned. What Youll Do Greets patients. Answer telephone calls and provide general information. Check-in and check-out patients. Schedule appointments and enter and track referrals. Scan and index documentation. Who You Are High School diploma or equivalent required. A minimum of one year of experience in a customer service position preferred.
