Licensed Professional Counselor or Licensed Marriage and Family Therapist

Eventus WholeHealth delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers, including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.

We are seeking a Therapeutic Behavioral Specialist to join our team! You will provide recovery-oriented support and case management services to residents of long-term care facilities and their families. Opportunities may exist for a future transition to providing counseling services. Our care addresses a wide range of issues, from anxiety and depression to severe mental illnesses and end-of-life concerns. As part of a multidisciplinary team, our clinicians significantly impact residents' quality of life by solving problems, delivering excellent clinical care, and serving as a vital link in the care of this vulnerable population.

Essential Duties and Responsibilities:

1. Provide Therapeutic Behavioral Services by consulting with a licensed clinician to meet individual needs.
2. Develop service plans for supports or care coordination, including healthcare and behavioral health treatment plans.
3. Implement linkage, interventions, treatment options, daily life skills restoration, and crisis prevention services.
4. Deliver services to individual clients or groups.
5. Train support staff on client triggers, de-escalation techniques, and interventions as per the treatment plan.
6. Foster positive relationships with outside contacts.
7. Meet with Resident Care Coordinators or designated staff to review resident and facility care needs and communicate findings.
8. Maintain open communication with facility staff and administration, providing verbal or written feedback on treatment plans and caregiver information.
9. Adhere to Eventus WholeHealth standards, policies, procedures, and all legal and regulatory guidelines, including HIPAA.
10. Conduct oneself professionally at all times.
11. Maintain patient confidentiality in all communications and records.
12. Consult with the clinical supervisor on unfamiliar resident or family requests regarding care.

Qualifications/Education:

Licensed Professional Counselors and Licensed Marriage and Family Therapists require a Master’s degree in counseling or marriage and family therapy, with eligibility for licensure in the practicing state.

• Population-specific interventions and care following treatment plans.
• Rules and regulations governing behavioral health practice.
• Organizational policies and procedures.

• Gathering and analyzing physiological, socioeconomic, behavioral, and emotional data.
• Providing training in social and emotional well-being, stress, anger management, and life skills.
• Evaluating patient problems and providing appropriate advice, interventions, or referrals.
• Effective written and verbal communication.

• Establishing and maintaining effective working relationships.
