This position actively manages patient care by coordinating and connecting patients with specialists and their facilities. The environment is fast-paced and rapidly changing and the philosophy revolves around primary care as the patient centered home and evidence-based medicine guidelines./ph3Job Responsibilities/h3ulliDelivers patient care, practicing under the oversight of an RN or clinic supervisor/liliCollaborates with patient and care team to facilitate a patient-centered medical home/liliProvides evidence-based nursing care at the highest community standard/liliCoordinates and connects the patient with specialists and their facilities/liliProvides phlebotomy services and immunization services/liliPerforms other duties as assigned/li/ulh3Required Qualifications/h3ulliLicensed Vocational Nurse from an accredited school with a minimum of 3 years of clinical experience/liliCurrent BLS (Basic Life Support) certification/li/ulh3Preferred Qualifications/h3ulliExperience in a fast-paced and rapidly changing environment/liliDemonstrated strong work ethic and takes pride in a detailed oriented approach to work/liliHighly organized and enjoys managing projects, big and small, to completion/liliCreative, flexible, and navigates through obstacles/liliExcellent follow-through with execution skills/liliAble to handle ambiguity and solving issues/li/ulh3Physical Job Requirements/h3ulliRequires standing, walking and sitting for extended amounts of time./liliOccasionally lift and carry items weighing up to 50 lbs./liliManual and finger dexterity and hand-eye coordination/liliIncludes full range of body motion including potential of handling and lifting patients./liliRequires corrected vision, hearing and speech within normal ranges.
