We are seeking a dedicated and compassionate Licensed Practical/Vocational Nurse to join our team as a Resident Care Coordinator at Silvis Center Rehab and Care. In this critical role, you will play a vital part in ensuring the highest level of care and support for our residents.

About the Role:

As a Resident Care Coordinator, you will be responsible for coordinating and implementing individualized care plans for our residents. You will work closely with our interdisciplinary team to ensure that each resident's unique needs are met, and that their care is delivered with the highest level of respect, dignity, and compassion. You will also be responsible for communicating with residents, families, and healthcare providers to ensure that all care needs are being met.

Responsibilities:

• Coordinate and implement individualized care plans for residents, including developing and implementing effective care strategies and interventions

• Assess and monitor resident needs, including physical, emotional, and social needs

• Communicate regularly with residents, families, and healthcare providers to ensure that all care needs are being met

• Participate in interdisciplinary team meetings to ensure that resident care is delivered in a comprehensive and coordinated manner

• Collaborate with other departments, such as social services and activities, to ensure that residents receive the best possible care

• Provide education and training to residents, families, and staff on various healthcare topics and care procedures

Requirements:

• Current LPN/LVN license and certification

• 1+ year of experience in a long-term care or rehabilitation setting

• Strong communication and interpersonal skills

• Ability to work effectively as part of a team

• Basic computer skills and familiarity with electronic medical records
