Occupational Therapist

Role Overview: An Occupational Therapist providing personalized, one-on-one care to older adults in private residences or Senior Living Communities, supporting mobility, independence, and quality of life through evidence-based treatment within our Geriatric House Calls™ model. Key Responsibilities Conduct evaluations, develop individualized care plans, and deliver therapy sessions to promote wellness and functional independence. Document patient progress accurately in the EMR system, adhering to regulatory standards. Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid Occupational Therapy license or eligibility in the practice state. Degree from an accredited OT program. Proficiency in computer use and EMR systems. Current CPR certification. Physical & Additional Requirements Ability to perform moderate physical effort, assist with patient transfers, and operate in home/community settings, including driving to appointments and wearing PPE when necessary. #J-18808-Ljbffr
