Job Summary:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team. As a hybrid Role, you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. This part-time position offers a unique blend of flexibility, great compensation, and a supportive clinical environment.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team

• Build strong relationships with facilities and maintain a patient-centered approach

• Collaborate with the clinical team to ensure seamless integration with facility processes and protocols

• Participate in ongoing education and training to stay current with industry developments and best practices

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Masters degree in Nursing or related field
