Care Coordinator Position at Lifekind HealthLifekind Health is looking for a Care Coordinator to join our team in Victorville. Ensure that patients receive comprehensive and continuous healthcare services tailored to their individual needs. Act as a liaison to facilitate communication among multidisciplinary teams, ensuring that care delivery is seamless and efficient. Clinics are open 7 days a week, 8:00 am - 7:00 pm.
