POSITION TITLE: Residential Case Manager-FT-Sunday/Monday off

PROGRAM/LOCATION: Community Residential Program San Luis Obispo

POSITION SUMMARY:

This position has a client caseload split between Community Housing and Bishop Street Studios. Case Managers for the Community Residential Program are expected to supervise the general health, safety, and social rehabilitation of clients. These residents will all be adults who receive services from San Luis Obispo Behavioral Health. They also perform moderate facility maintenance, function as positive role models, promote cooperation and harmony in communal living, and uphold and promote the program philosophy in all duties performed. The overall goal of the program is to help adults with serious mental illness succeed in the community with sufficient structure and support. The Case Manager will be responsible for crisis care, individual rehabilitation activities, day-to-day skills building, transportation to appointments, and all other aspects required to help the clients maintain stability. The Case Manager will work Tuesday through Saturday providing a full range of services to the residents of CRP using a trauma informed approach. The Case Manager will work at the CRP SLO office Tuesday through Thursday and the Bishop SLO Office Friday and Saturday.

HOURS/ SALARY EMPLOYEE PERKS:

Full-time, non-exempt position. Tuesday through Saturday with Sunday and Monday off. 40 hours per week. Anticipated hiring range is $25.50-$26.50. Transitions Mental Health Association is a nonprofit organization dedicated to eliminating stigma and promoting recovery and wellness for people with mental illness through work, housing, community and family support services.

RESPONSIBILITIES AND DUTIES:

1. Provide case management for an assigned group of clients living in Community Housing and at Bishop Streets Studios. Case Management duties will include one-on-one support to the residents as needed; providing symptom management support; educating and supporting residents families while acting in the residents best interest and advocating for the rights of the residents. Assisting residents in all aspects of independent living including the development of social, vocational, recreational and effective life management skills; assisting clients in the areas of proper nutrition, cooking, shopping, budgeting, housecleaning, and maintenance, and personal hygiene.

2. Provide ongoing assessment of residents psychiatric symptoms and their responses to treatment. Recognize and be available to clients in psychiatric crisis or emotional distress, and assist clients in developing appropriate coping and conflict resolution skills. Provide counseling, emotional support, and symptom recognition and management. Educate clients and about the nature of mental illness, symptoms of mental illness, coping strategies, and medication. Assist clients in developing ways to manage symptoms, augment coping skills, and strategize problem-solving skills.

3. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, individually and in team meetings, the Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.

4. Assist clients in learning more about substance abuse and treatment options open to them such as A.A. and other support groups. Work with clients in identifying possible precursors that could lead to alcohol or drug abuse.

5. Assume the role of advocate in all aspects of clients needs and goals. This duty may include familiarizing clients with and encouraging the usage of community resources, support systems, housing opportunities, medical services, and opportunities; and assisting clients in building healthy peer and familial support networks. Assist clients in developing various recreational skills.

6. Discuss with clients what goals they plan to achieve for them-selves and assist clients in determining, developing, and carrying out their own goals; review plans and goals, evaluate progress, and strengthen independence.

7. Provide ongoing assessment of activities of daily living. Assist clients in developing skills in the following areas, as needed, in order to achieve the highest level of independence possible: personal hygiene, shopping and cooking skills, money management skills, accessing reliable transportation, and scheduling / attending appointments.

8. Provide opportunities to allow clients to establish vocational goals and identify any job-related areas that they want to work on. Provide vocational counseling, referral to appropriate vocational services, and assist clients with job coaching and support as needed.

9. Act as agency liaison by consulting, collaborating, and communicating with Program Manager, SLO County Behavioral Health, Housing Authority of San Luis Obispo and other treatment agencies to meet client and program objectives. This may include coordinating referrals and/or providing information to other agencies or treatment programs.

10. Supervise the cleanliness and upkeep of the residences; this duty may include general household chores, reporting of needed repairs, acquisition and disposal of furniture, and teaching and assisting clients in maintaining a clean and sanitary living environment. Complete monthly facility checks at each unit on your caseload.

11. Complete program paperwork; this may include individual client files, admission/ discharge paperwork, petty cash receipts and vehicle checklists.

12. Medi-Cal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Program Manager and Director.

13. All direct service staff are expected to spend at least 50% of their work time in direct service and support of clients, unless otherwise assigned by your supervisor.

14. Attend regular staff meetings and complete required hours of in-service training.

15. Assume other duties and responsibilities as assigned by the Program Manager, Program Director and Assistant Program Manager.

JOB ENVIRONMENT:

1. Daily and frequent documentation of client services on the computer.

2. Moderate, physical activity and frequent driving; may include moving or lifting up to 50 lbs.

3. Involves moderate risks that require safety precautions and the risk of exposure to potentially intense situations.

4. Position involves working within the residential environments.

5. Possible intense situations related to confrontational or oppositional adults.

Requirements:

Minimum Requirements:

1. BA/BS AND 6 months of full-time equivalent experience with target population, OR AA/AS or CADAC/CPRP certification AND 2 years of full-time equivalent experience with target population, OR 4 years of full-time equivalent experience with target population.

2. Must have a strong commitment to the right, ability and choice of each person to live in a safe environment; develop a meaningful role in the community; and have access to competent and continuous supports and services.

3. Must have a reliable automobile and utilize own vehicle at work with company mileage reimbursement when a company vehicle is not available.

4. Strong interpersonal skills, collaborative work style and strong oral/written and communication skills.

5. Well defined organizational and time management skills.

6. Ability to work independently and flexibly.

7. Ability to handle intense situations, display good judgment and decision-making skills, and remain calm and clearheaded under pressure.

8. Able to work effectively with a variety of people and personality types regardless of age, ethnicity, race, religion, sexual orientation, gender identity/expression or socio-economic level.

9. Ability to possess and maintain good physical and mental health.

10. Ability to work well within a team and maintain a professional, respectful attitude in the workplace.

11. Ability to be willing to work with supervisor oversight and direction.

12. Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees.

17. Conditions of employment include:

Must be at least 18 years of age; possess a valid CA Drivers License, at least 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHAs current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance.

Must comply with a TB assessment (possibly a TB test if needed to confirm a negative tuberculosis result) and receipt of a criminal record clearance through fingerprinting.

Must be able to successfully complete First Aid and CPR training
