Pay: $18-$20/hr. Patient Care Coordinator Duties
- Coordinate patient admissions for home infusion and infusion suite services.
- Serve as the primary liaison between patients, physicians, nurses, pharmacists, and other healthcare professionals.
- Obtain, review, and process physician orders, referrals, and supporting medical documentation.
- Verify insurance benefits and assist with prior authorizations and coverage requirements.
- Schedule patient appointments, infusion services, and supply deliveries.
- Maintain accurate patient records and ensure documentation meets regulatory and company standards.
- Educate patients and caregivers about services, treatment plans, and care processes.
- Monitor ongoing patient needs and communicate updates to the clinical team.
- Address patient questions, concerns, and service issues while providing excellent customer service.
- Coordinate start-of-care activities to ensure timely initiation of therapy.
- Work closely with intake, pharmacy, nursing, and billing departments to support continuity of care.
- Assist with insurance, billing, and reimbursement-related communication when needed.
- Customer service and patient relations
- Medical terminology knowledge
- Insurance verification and authorization experience
- Strong organizational and multitasking abilities
- Electronic medical records (EMR) proficiency
- Phone and communication skills
- Attention to detail and documentation accuracy
