We're a Social Primary Care practice serving clinically and socially complex older adults in underserved Boston communities. Our model gives you the time, the team, and the tools to actually treat the whole person not just their chart.

Panel size is capped at 750 patients.  Full stop.

A real care team behind you You're the clinical quarterback, but you're not working alone. Your team includes:

• Gather Guides community-based partners who address social determinants of health and connect patients to resources
• Medical Technician Partners (EMTs/Paramedics)deployed into patients' homes for post-discharge follow-ups, chronic condition monitoring, and home safety assessments, reporting directly back to you
• Medical Assistants, Patient Benefits Partners, and Practice Operations Partners handling the administrative and clinical support so you can focus on care

Schedule flexibility built in Administrative days can be completed remotely. We're designed to support sustainable, long-term practice not burnout.

Value-based care, not fee-for-service volume Our model frees you from the productivity treadmill. Competitive base salary plus performance-based incentives aligned with patient outcomes not visit counts.

Patients who benefit most from what you do best Our patients are older adults navigating complex medical and social challenges. This is longitudinal, relationship-driven primary care with real continuity in our state-of-the-art centers, via telehealth, and in patients' homes.

A typical week

• Clinic and telehealth visits with your panel (no double-booking, no 10-minute slots)
• Care team huddles with Guides and Medical Technician Partners
• Remote administrative time for documentation, care coordination, and team communication
• Home-based care coordination supported by your MTP team (you direct; they deploy)

What we're looking for

Required:

• Licensed Nurse Practitioner or Physician Assistant with primary care experience
• Active Massachusetts medical license, or ability to obtain one prior to start
• Clinical proficiency in evidence-based primary care
• Ability to connect authentically with patients from diverse backgrounds
• Comfort with EHR and care technology platforms
• Strong ethics and discretion in complex, sensitive situations
• Collaborative spirit we work without hierarchy and welcome all perspectives

Preferred:

• Experience caring for older adults in underserved communities with complex social challenges
