In-Home Primary Care Physician Assistant
Signify Health
Signify Health seeks a mission-driven Physician Assistant to perform in-home clinical evaluations for health plan members. You will conduct comprehensive assessments, review histories, perform focused exams, and document findings using digital tools to support value-based care. Collaborating with an interdisciplinary team, you’ll identify care gaps, reinforce care plans, and provide patient education to improve outcomes for vulnerable populations.
