Nurse Care Manager

Elevance Health

Elevance Health is seeking an experienced registered nurse to perform care management activities for members with complex and chronic conditions, in collaboration with providers and care teams. This role emphasizes individualized care planning and outcomes. The position is 100% remote with possible contract-to-hire, requires active RN license, multi-state licensure, and Nevada/Connecticut/Missouri compacts, with live go-live 9/1.
