Nurse Practitioner - Health Risk Assessments

$115 - $125 per unit

Seeking experienced Nurse Practitioners with over 1 year of experience to perform In-Home Health Risk Assessments . Responsibilities include conducting comprehensive evaluations for Medicare Advantage patients, reviewing medical histories, and assessing social factors to improve care quality and address care gaps during home visits. Qualifications include being a board-certified Nurse Practitioner licensed in Florida and having home health experience . Key duties involve setting flexible hours within a self-directed schedule, covering at least 3 counties, and earning $115-125 per assessment, with additional mileage reimbursement and no-show payments.
