Nurse Practitioner - Spanish Speaking

Overview ChenMed is a physician‑led, mission‑driven primary care organization that serves Medicare Advantage beneficiaries. We are looking for a dedicated Primary Care Provider (Nurse Practitioner) to join our team and help us advance patient care for seniors. Responsibilities Independently provide comprehensive care to up to 450 patients, managing acute and chronic illnesses. Assume full accountability for patient outcomes, including assessment, diagnosis, treatment, education, health promotion, and care coordination. Serve as a health coach, setting short‑ and long‑term health goals and following up to ensure progress. Simplify and prioritize care actions to make behavior change actionable for patients. Collaborate with hospitalists, case managers, transitional care teams, and other health professionals as needed. Supervise the care team (medical assistant, facilitator, coordinator) and step in to cover other providers when necessary. Participate in risk and quality management programs, clinical meetings, and regulatory compliance activities. Lead efforts to recruit patients and additional providers for the center. Knowledge, Skills & Abilities Availability: Provide timely walk‑in visits, phone calls, and messages to build patient trust. Service Orientation: Deliver care that one would want for a family member. Evidence-Based Medicine: Stay current with clinical, nutritional, and lifestyle interventions. Leadership: Continuously develop leadership skills for patients, team, center, and company. Quality Focus: Work toward high‑quality outcomes and continuous practice improvement. Influence & Compassion: Effectively influence patients and team members to achieve optimal results. Self‑Care: Maintain personal well‑being to provide the best patient care. Technical Proficiency: Use Microsoft Office Suite and other relevant software. Travel: Travel locally, nationally for training, and regionally/nationally for recruiting or training as required. Language: Fluent in English; other language fluency may be required depending on location. Education & Experience Current Nurse Practitioner certification in the state of practice. Certification in Basic Cardiac Life Support. Board certification by AANP or ANCC preferred; may be required in certain states. DEA number for controlled substances (Schedule II‑V) may be required. 1 year of clinical experience preferred; strong new graduates considered. Compensation Pay range: $111,140 – $158,771. Final compensation depends on experience, education, location, and other factors. The position may also qualify for bonuses or commissions.
