Position Overview (Certified Nursing Assistant – CNA)

Polaris Rehabilitation & Care Center is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to provide direct, hands-on care to residents in our skilled nursing environment. This role is essential in supporting residents with activities of daily living while promoting dignity, safety, comfort, and overall well-being.

As a CNA, you will work closely with licensed nursing staff and interdisciplinary team members to deliver high-quality, resident-centered care in accordance with individualized care plans, facility policies, and applicable regulatory standards.

Key Responsibilities

• Obtain, record, and report resident vital signs, including temperature, pulse, respirations, and blood pressure

• Promptly communicate abnormalities or changes in resident condition to licensed nursing staff

• Assist residents with mobility, including transfers, ambulation, repositioning, and the use of mobility equipment

• Provide support with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and personal hygiene

• Respond promptly and professionally to call lights and resident requests for assistance

• Observe and report changes in resident behavior, mood, mobility, skin condition, appetite, or overall well-being

• Accurately document care provided in accordance with facility procedures and regulatory requirements

• Maintain a clean, safe, and organized resident environment consistent with infection prevention and safety standards

• Follow fall prevention, skin integrity, transfer, and resident safety protocols

• Communicate respectfully and effectively with residents, families, visitors, and interdisciplinary team members

• Promote resident dignity, privacy, independence, comfort, and quality of life

• Adhere to facility policies, resident rights, infection prevention standards, and applicable federal and Wyoming regulations

Qualifications

• Current, active Certified Nursing Assistant (CNA) certification in good standing in the State of Wyoming

• No disqualifying findings on the applicable Nurse Aide Registry

• Ability to perform the essential functions of the position with or without reasonable accommodation

• Previous skilled nursing, rehabilitation, long-term care, or healthcare experience preferred

• Strong observation, communication, and interpersonal skills

• Ability to work effectively within a collaborative clinical team

• Ability to adapt and prioritize responsibilities in a fast-paced healthcare environment

Physical Requirements

• Ability to lift, transfer, reposition, and assist residents safely, including up to 50+ pounds with assistance and equipment as needed

• Ability to stand, walk, bend, push, pull, and perform physical care duties for extended periods

• Ability to safely operate resident transfer and mobility equipment

• Ability to respond promptly and effectively to resident needs and emergency situations
