Provides comprehensive psychosocial assessment and support to patients and their families across all age groups. Delivers crisis intervention, counseling, and makes appropriate referrals while collaborating with interdisciplinary teams to enhance patient care. Responsibilities:
- Evaluate patients' psychosocial needs and develop individualized care plans.
- Conduct crisis interventions and provide bereavement counseling as needed.
- Collaborate with community agencies and healthcare teams to coordinate resources.
- Offer consultation and training to staff on psychosocial issues and interventions.
- Participate in departmental meetings and interdisciplinary activities.
- Maintain accurate documentation of services provided and patient progress.
- Hold a Bachelor's Degree or higher in social work or related field.
- Possess a Master's Degree in social work or equivalent combination of education and experience.
- Have at least three years of experience in a healthcare or social services setting.
- Be licensed or eligible for licensing as an LCSW or equivalent credential.
- Maintain valid CPR certification (such as BLS).
- Have current CPI certification or complete training onsite if required.
