Responsibilities
- Evaluate residents’ mobility, balance, strength, and functional abilities to develop individualized PT plans of care.
- Implement and adjust treatment plans, including therapeutic exercises, manual therapy, and equipment use, to maximize functional independence.
- Reassess progress regularly, document outcomes, and update goals in the patient record.
- Collaborate with an interdisciplinary team (nursing, OT, SLP, and case management) to optimize rehabilitation outcomes.
- Educate residents and families on home exercise programs, safety strategies, and caregiver training to support continuation of care post-discharge.
- Ensure compliance with professional standards, facility policies, and infection control protocols.

Requirements
- Active Physical Therapist license in the United States with eligibility to practice in Maryland.
- Experience or demonstrated interest in skilled nursing facility or geriatric rehab settings.
- Ability to meet guaranteed hours of 38 per week during the assignment.
- Strong communication and documentation skills; ability to work within an interdisciplinary team.
- Facility-based, patient-centered approach with reliability and professionalism.
