Job Title: Registered Nurse (RN) Case Manager - Home Health1:1 Care, Autonomy, Flexible Field Role

Company: Home Health Care, Inc.

Location: Greater Twin Cities Metro Area (21-county service area)

Schedule: Full-Time, Monday to Friday, 8:00 AM - 5:00 PM, 1 weekend per month, 1 on-call shift per week

Compensation: $85,000 - $130,000 annually plus $6,000 in first-year bonuses

What You'll Do
•Manage a home health caseload across homes and community settings.
•Complete OASIS assessments and individualized care plans .
•Provide skilled nursing care, education, and care coordination .
•Work closely with physicians, therapists, and interdisciplinary teams.
•Support patients in improving function, safety, and independence .
•Maintain accurate, timely documentation .

What We're Looking For
•Active Minnesota Registered Nurse (RN) license.
•3+ years of RN experience, home health preferred.
•Strong clinical judgment, assessment, and documentation skills .
•Ability to work independently in a field-based role .
•Reliable transportation.
