Responsibilities include: Perform initial, comprehensive, and ongoing nursing assessments as assigned. Develop, implement, and evaluate individualized plans of care in collaboration with the physician and care team. Provide skilled nursing assessments, treatments, and interventions according to orders and the plan of care. Administer medications and treatments within the scope of practice and applicable orders. Monitor vital signs, assess patient conditions, and identify changes requiring clinical follow-up. Educate patients and caregivers on medications, disease management, treatment plans, and home safety. Coordinate care with physicians, therapists, case managers, caregivers, and other healthcare professionals. Complete accurate, timely clinical documentation in the agency’s EMR system. Complete required assessments and OASIS documentation when applicable and within assigned responsibilities. Maintain compliance with HIPAA, agency policies, and applicable state and federal regulations. Participate in required training, in‑services, and quality improvement activities. Deliver compassionate, patient-centered care that promotes dignity, safety, and independence. Qualifications Current, unrestricted Florida Registered Nurse (RN) license required. Home health nursing experience preferred. OASIS experience strongly preferred. Experience with electronic medical records; Axxess experience is a plus. AHCA Level II background screening. Current CPR certification. Valid driver’s license, reliable transportation, and current auto insurance. Strong clinical assessment, critical‑thinking, and documentation skills. Excellent written and verbal communication. Ability to work independently while maintaining effective communication with the clinical team. Compassionate, dependable, and patient‑focused approach. Experience with geriatric patients, Veterans, medically complex patients, acute care, long‑term care, or hospice and palliative care is a plus.
