Job Description The Occupational Therapist-Home Health in Northern AZ plays a key role in delivering personalized therapy services to primarily adult patients within their homes. As part of an interdisciplinary team, this position involves frequent travel to multiple residences, focusing on patient assessment, care planning, and implementing effective therapeutic strategies to enhance patients’ daily living skills. The therapist also educates patients and caregivers, monitors progress, and ensures safety through appropriate recommendations and documentation.
