Schedule Monday - Friday (30 hour weekly) Day shift only Key Responsibilities Must be able to function at a high level with little oversight to provide nursing care, training, and home support (as needed) Works collaboratively with Dialyze Direct team members, skilled nursing home facility representatives and dedicated Nephrologists Collects and records relevant patient health data in the course of/pre/post hemodialysis treatment Develops and maintains a plan of care that describes interventions to attain expected outcomes Implements and records interventions to attain expected outcomes in EMR Performs laboratory collection, processing, and shipping procedures and reschedules missed or inadequate laboratory collections Perform clerical tasks in addition to patient care (i.e., filing, inventory management, admission documentation, creating patient charts, etc.) Maintains a positive and respectful attitude to all patients and colleagues Occasional travel to other nearby locations Medication administration and documentation Requirements Valid RN license 6 months of RN experience strongly preferred 3 months of hemodialysis experience Valid drivers license and transportation Ability to travel covering multiple locations BLS certification (American Red Cross or American Heart Association Required) Ability to lift up to 35 pounds is required This is a safety sensitive position Benefits 401(k) package Flexible Spending Accounts Education Assistance Certification & Professional Membership Reimbursement Compensation Salary Range: $40.00 - $60.00 per hour.
