MDS Coordinator

Clement Manor is a faith-based independent living, assisted living, CBRF and long term care community, nationally recognized for providing a superior quality environment for older adults. We are currently looking for a Full Time MDS Coordinator. This position is part of the on-call rotation with every 5 week obligation and included in the on-call holiday rotation. The MDS Coordinator oversees and coordinates the development and completion of the resident Assessment ( MDS) in accordance with current Federal and State rules, regulations, and guidelines that govern the resident assessment, including the CAA’s. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements represent the knowledge, skill and/or ability required. Must be a graduate of an accredited School of Nursing, including current licensure by the state of Wisconsin and current AANAC certification preferred but not required. Must possess the understanding and ability to comply with Federal and State rules and regulations as it applies to Nursing Homes, such as PDPM. Equipment operation: Phone, stethoscope, computer copier, printer, scanner and other nursing equipment. Strong Assessment skills Ability to facilitate and communicate with all disciplines. Ability to organize and prioritize Knowledge of computer applications and processes
