LTSS Service Care Manager

Job Title

Location: Delaware

Work Mode: Hybrid (mixture of care visits and remote work)

Shift: 8am – 5pm EST Mon – Fri, No OT expectations

Duration: 6 months, potential to extend or convert

Responsibilities:

Complete LTSS level of care redetermination
Conduct a mixture of care visits and support other LTSS case managers
Support the case management team due to a high increase in membership

Requirements:

Bachelor's degree
Minimum 2 years of related experience
Delaware/Compact RN License
Driver's license
Home workspace
Experience in Level of Care Redetermination, Transition of Care, and Case Management
Computer literacy
Experience evaluating the level of functioning of older adults/disabled individuals
Experience with medication reconciliation

Preferred Skills:

Service Care Manager experience
