Remote Spanish-Speaking Health Coach & Navigator

Cadence

Cadence seeks a bilingual Spanish-speaking Clinical Navigator to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct outreach, coach patients on goal setting, educate on disease management, and coordinate care with clinicians using Cadence Platform, Google Workspace, and Slack. This is a full-time remote role based in the United States. The ideal candidate has NBHWC certification, 1+ year health coaching experience, and comfort #J-18808-Ljbffr
