Meadow Falls of Bath is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in resident condition to ensure timely interventions

• Utilize strong communication skills to effectively communicate with residents, families, and healthcare team members

• Maintain accurate and detailed records of patient care and treatment

• Participate in quality improvement initiatives to optimize patient outcomes

Requirements:

• Current LPN licensure in the state of Ohio

• LPN (new graduates welcome to apply)

• Ability to work variable shifts, including days, evenings, and weekends

• Strong communication and interpersonal skills

• Ability to lift and transfer residents as needed
