The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately.  The Clinical Supervisor is responsible for ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel and Care Coordinators.

Essential Job Functions/Responsibilities

• Provides guidance and counseling to Care Coordinators to assist them in continually improving all aspects of home health care services.
• Manages and coordinates the Care Coordinators teams and planning
• Oversees/ assists in evaluating the performance of clinicians.
• Maintains current knowledge related to Medicare Conditions of Participation and the Accreditation Commission for Health Care (ACHC) standards to ensure that the organization is compliant with state, federal and accreditation guidelines.
• Acts as a clinical resource to personnel.
• Maintains updated nursing knowledge and skills.
• Assists with patient visits as needed
• Responsible for the follow-up of concerns expressed by patients, families, and physicians.
• Collaborates with Director to provide a process to promote and ensure employee orientation, competency and inservice education programs.
• Assists with administrative call on a rotational basis and clinical call as needed.
• Participates in Quality Improvement and Corporate Compliance activities as assigned.
