Come join our growing team! The Care Team Hospice is looking for a Full-Time CNA in Cleveland, Ohio and surrounding areas. Primary hours for this position are Monday - Friday 8:00am - 4:30pm.

Qualifications

Key Responsibilities

• Provide personal care, hygiene, and comfort measures
• Observe and report changes in patient condition
• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)

Qualifications

• Active CNA certification
• Hospice aide experience preferred
• Experience using HCHB (Homecare Homebase) preferred

Field Role Requirements

• Active driver's license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
