Remote Health Care Coordinator

Oversee and review patient care for house call patients. Assess medical records and develop individualized care plans. Coordinate patient visits with House Call Team. Reconcile medications and coordinate prescriptions with pharmacies. Review labs, diagnostics, and order tests or referrals as needed. Communicate with patients, caregivers, and healthcare teams. Ensure compliance with clinical standards and organizational policies. Monitor and address care gaps for non-compliant or high-risk patients. Qualifications Medical Doctor (MD) with a valid and active medical license. Previous clinical or patient care experience preferred. Familiarity with Electronic Health Record (EHR) systems and medical documentation. Knowledge of care coordination, patient services, and healthcare operations. Strong communication and interpersonal skills when interacting with patients and healthcare teams. Ability to manage multiple tasks, coordinate schedules, and support clinical workflows. Strong attention to detail and ability to maintain accurate patient records. Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality. Ability to work in a fast-paced healthcare environment and collaborate effectively with multidisciplinary teams.
