Job Title: LPN Patient Care Coordinator - Home Care/HospiceLocation: Rochester Regional Health Home Care - Monroe Ave, 330 Monroe Avenue, Rochester, NY 14607Hours Per Week: 40Schedule: Hybrid Schedule - 8:30am - 5pm, Monday/Friday - Remote & Tues/Wed/Thurs - in Monroe officeSummary:The Patient Care Coordinator delivers superior customer service while receiving and processing referrals initiated at the customer care center point of entry for Rochester Regional Health Home Care and Hospice. Performs all functions that facilitate the efficient processing of referrals and promotes overall patient satisfaction with Rochester Regional Health Home Care and Hospice Services.Responsibilities:Screens all inquiries from referral sources (hospitals, physicians, clinics, patients/family members, etc.) and provides accurate and reliable information regarding the criteria for admission to home health care/hospice services.Responds knowledgeably and professionally to inquiries regarding Rochester Regional Health Home Care and Hospice services and programs.Obtains necessary patient specific information at the time of referral to maximize the potential that patient will meet admission criteria.Appropriately and accurately inputs admission data and completes referral and coordinates service requests with a focus on the patients’ care needs and to assure a timely admission to service.Obtains required billing information to process verification and billing authorization and generates physician orders.Understands and utilizes Medicare home care guidelines in establishing home care referrals.Coordinates all patient specific needs, i.e., scripts to vendor for IV needs, DME Requests or other tasks as assigned.Answers calls and obtains pertinent information to quickly and accurately identify and assess caller’s needs and determine action steps to satisfy those needs or to resolve basic types of customer service inquiries.Projects patience, empathy, caring and sincerity when answering phones and ends all calls with “Is there anything else I can do for you today”.Collects, documents and provides reports of required data,Builds and maintains productive working relationships with all internal and external customers.Provides assistance when needed with referral process for Home Care Coordinators.Provides back up and support for all special assignments or tasks assigned to other Patient Care Coordinators.Assists with special projects, activities.Consistently demonstrates high standards of integrity by supporting the Rochester Regional Health's mission and values and adhering to the Corporate Code of Conduct.Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.With training provided, must demonstrate the ability to learn and utilize new systems, computer applications and operating environments as necessaryRegular and reliable attendance is expected and required.Performs other functions as assigned by management.PREFERRED QUALIFICATIONS:2 years home health care LPN experience preferred.Maintains knowledge of insurance and reimbursement guidelines and changes.Strong communication skills. Strong decision-making and problem-solving skills.Excellent time management and organizational skills.Ability to handle changing work environment and to prioritize activities.Proven ability to work in a team environment.Flexibility to work a varied work schedule when needed to meet department needs.Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents if necessary.REQUIRED QUALIFICATIONS:Graduate of an accredited Licensed Practical Nursing program with a minimum of 2 years LPN experience required.A minimum of 2 years customer service experience required.Required Licensure/Certification:Valid NYS LPN licenseEDUCATION:LICENSES / CERTIFICATIONS:PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.Any physical requirements reported by a prospective employee and/or employee’s physician or delegate will be considered for accommodations.PAY RANGE:$25.00 - $31.00CITY:RochesterPOSTAL CODE:14607The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
