Hospice Registered Nurse Case Manager

Role Overview This position involves providing bedside nursing care as a Hospice Homecare Case Manager Registered Nurse, focusing on delivering compassionate post-acute services, including hospice, home health, and rehabilitation. The role emphasizes collaboration to improve clinical outcomes, support patients' needs, and ensure quality care delivery. Responsibilities Assess patient conditions and develop individualized care plans. Coordinate and communicate with interdisciplinary teams, patients, and families. Administer treatments, monitor patient progress, and document care activities. Ensure compliance with healthcare standards and safety protocols. Provide education and emotional support to patients and families. Qualifications & Skills Current RN license with at least one year of experience in healthcare. Basic Cardiac Life Support certification within 30 days of employment. Valid driver's license within 30 days of employment.
