Responsibilities Under the supervision of the RN Manager, act as a liaison between high-risk patients and the medical care team/provider Work with patients for a pre-defined period, conducting telephonic assessments and documenting/communicating findings to the medical care team Assist patients with scheduling appointments and preventive screenings Help patients find applicable community resources and coordinate with vendors as necessary Maintain clear and timely communication with the care team and other healthcare personnel Qualifications Required Qualifications High School diploma or GED and graduation from an accredited Medical Assistant program or school; 3+ years of experience in a health care insurance company or medical clinic EMR use experience Good alpha/numeric data entry skills with minimal errors Proficient with computer applications including Microsoft Word and Excel Proven multitasking, problem solving, and analytical skills Detail oriented with strong verbal and written communication, interpersonal skills, and ability to work in a fast-paced environment Strong organizational skills Ability to communicate and collaborate with colleagues, physicians, nurses, health plan staff, and other healthcare personnel Ability to communicate with physicians regarding patient needs and with all levels of management Ability to prioritize and work independently Preferred Qualifications Medical Assistant Certification Certified or Registered Medical Assistant CPR/BLS or other relevant healthcare certification In-person training at Pearland up to 90 days depending on progress 2+ years of experience with customer service or outbound calls Experience with home health or DME Medical background such as Health Coach or Community Health Worker Proficient with EPIC EMR Remote role with some in-office requirements (meetings and training) Quiet, secure home workspace and availability during business hours Solid home internet connection for remote work Adherence to UnitedHealth Group’s Telecommuter Policy for remote employees Compensation and Benefits Pay is based on factors including local labor markets, education, work experience, and certifications. The hourly pay range for this role is $16.15 to $28.80 depending on full-time status. In addition to salary, a comprehensive benefits package, incentive programs, equity stock purchase, and 401k contribution are offered where eligible.
