Job Summary and Qualifications

In this role, you will be responsible for delivering direct patient care in alignment with physician directives, adhering to established standards of practice, and following organizational policies and procedures. You will implement the care plan, administer treatments, and collaborate with team members to achieve favorable patient outcomes.

Your responsibilities will include:

• Implementing the care plan, administering treatments, and collaborating with team members to ensure positive patient outcomes.
• Providing direct patient care as outlined in the State Nurse Practice Act, including specialized responsibilities related to end-of-life care.
• Coordinating hospice services and effectively communicating identified needs to the interdisciplinary team, including Hospice Aide, Director of Clinical Services – Hospice, and Chaplain.
• Managing pain and symptoms appropriately.

Qualifications required:

• Basic Cardiac Life Support certification must be obtained within 15 days of starting employment (for RNs).
• Must be a Registered Nurse.
• Possess an Associate Degree or Bachelor's Degree.
• A minimum of 1 year of relevant experience is required.
