Physician Job Requirements

• Graduate of an accredited MD or DO program
• At least 5 years of physician or clinical experience
• Valid Indiana physician license

• Internal Medicine experience preferred
• Ability to complete required tuberculosis screening for this patient-facing role
• Strong commitment to patient-centered care and continuous improvement of the patient experience

Senior Primary Care Physician Responsibilities

• Provide comprehensive outpatient primary care for senior and geriatric patients
• Conduct wellness visits, physical examinations, health screenings, and preventive care evaluations
• Diagnose and manage acute illnesses, chronic conditions, and minor injuries
• Develop individualized treatment plans and coordinate care with specialists when appropriate
• Maintain accurate medical histories and timely clinical documentation
• Support continuity of care through strong, lasting patient relationships
• Address the physical, emotional, social, and behavioral factors that influence patient health
